Position: Permanency Worker 

Department: Child Welfare  

Reports To: Case Manager Team Supv.  

FLSA Code: Exempt

Hours: 40 hours per week (specific hours are flexible)

GENERAL ACCOUNTABILITY:

Under close supervision provide direct services to children, natural parents and their foster parents.  Provide adoption services to foster parents. 

SPECIFIC ACCOUNTABILITIES: 

  1. Assist workers in completing all items on the screening checklist; which consists of requesting termination of parenting rights for each child, birth certificates, death certificates, surrenders, service plans and social histories. 
  2. Worker is responsible for completing a complete adoption subsidy and the legal documents necessary. 
  3. Worker attends courts and provides information to t he court about the nature and progress of the case, services that are provided to the family and the welfare of the child. 
  4. Worker will prepare families for adoption and provide support. 
  5. Worker will formalize tools and techniques for the home assessment process. 
  6. Worker will maintain once a month home visits and be responsible for obtaining school repots, medical and dental records. 
  7. Worker will submit case closing information to Juvenile court and then prepare the case closing, subsidy within five working days of the finalized adoption. 
  8. Worker will complete subsidized guardianship transfer packets and submit to DCFS.  Worker will complete adoption closing information and subsidy opening information and submit to DCFS for all adoption cases. 
  9. Worker will complete statistical reports for all children placed and adopted during the month. 
  10. Worker will communicate all procedures of the cases to the Supervisor. 

QUALIFICATIONS

Education Experience:

 Bachelor’s degree from an accredited university in a human service field.

Training and Skills:

Two years of foster care case management preferred

  • Ability to work collaboratively with other staff, service providers and professionals. 
  • Ability to set limits and intervene appropriately to meet the needs of persons served.
  • Ability to utilize trauma informed approach with all clients.
  • Conviction about the capacity of people to grow and change. 
  • Ability to value a nurturing family as the ideal environment for a person.
  • Sensitivity to the service population’s cultural and socioeconomic characteristics.

Work Requirements:

  • Pass a CANTS (Child Abuse and Neglect Tracking System) check.
  • Pass a Criminal Background check.
  • Pass 402, CCA and Case Manager Licensing examination with 60 days of hire 
  • Maintain confidentiality
  • Attend regular staff meetings 
  • Possess and maintain a valid Driver’s License and possess safe and reliable transportation.
  • Possess and maintain adequate Liability Insurance for the vehicle.

Job Skills:

  • Ability to provide for the individual needs of children 
  • Ability to provide help and support for foster parents of children in placement  
  • Ability to work effectively with parents of children in placement.
  • Ability to locate and utilize resources for all individuals in the child welfare system 

WORK ENVIRONMENT

The typical work environment is in an office setting, but may on occasion be outside in a sometimes threatening and hostile environment.  The noise level will vary depending on the location.  External vehicles, office equipment, machinery, telephone and conversations produce level.
