Position: Permanent Housing Case Manager

Department: Housing

Reports To: Director of Permanent Housing

FLSA Code: Exempt

Hours: 40 hours per week (specific hours are flexible)

GENERAL ACCOUNTABILITY

Provide intensive case management services to individual and families in the program under supervision of

Permanent Housing Director.

SPECIFIC ACCOUNTABILITIES

1. Meet with adult family members at least monthly.

2. Develop and implement a service plan for each individual/family with goals and objectives geared

toward maximum independence. Ensure that goals incorporate the strengths and weaknesses of the

individual/family served. Review the service plan at least bi-annually and assess progress toward

meeting desired goals.

3. Provide or refer participants with needed services to include: psychiatric evaluation/treatment, substance abuse evaluation/treatment, inventory of competencies, skills inventory, employment counseling/placement, medical services, educational/job training resources, vocational training, public legal services and housing resources and referral.

4. Facilitate effective utilization of all community resources to help participants achieve the goals outlined in their service plans.

5. Provide short term crisis intervention to individuals and families as needed. Consult with supervisor as to nature of and duration of service.

6. Conduct periodic family staffings under supervision to determine service needs and progress toward meeting goals.

7. Complete written documentation on all relevant services. Ensure proper placement in the case file. Supervisor must review and sign off on all documentation.

8. Provide follow-up (After-care) services, as needed.

9. Actively participate in the agency’s Program Quality Improvement (PQI) Plan.

10. Entering client information into the Housing Management Information System (HMIS) for Permanent Housing on a regular basis.

11. Update job knowledge by participating in educational opportunities, completing required web based/relias training, reading professional publications and participating in professional organizations.

12. Maintain written eligibility criteria with outlines on who is eligible for service and ensure that the criteria is met prior to program entry. Contact coordinated entry systems to obtain required documentation and outreach to referred person(s).

13. Ensure that client services are coordinated internally and with outside agencies.

14. Ensure that resources such as utensils, food, bedding, blankets and other basic necessities are adequate and available to meet the needs of the clients.

15. Participate in supervision with Program Director at least bi-monthly to ensure services conform to Unity’s, funders and COA policies and standards.

QUALIFICATIONS

Education Experience:

Bachelor’s degree from an accredited university in a human service field.

Training and Skills

One year working with homeless families preferred

Worker Traits:

• Ability to work collaboratively with other staff, service providers and professionals.

• Ability to set limits and intervene appropriately to meet the needs of persons served.

• Ability to utilize trauma informed approach with all clients.

• Conviction about the capacity of people to grow and change.

• Ability to value a nurturing family as the ideal environment for a person.

• Sensitivity to the service population’s cultural and socioeconomic characteristics.

Work Requirements:

• Pass a CANTS (Child Abuse and Neglect Tracking System) check.

• Pass a Criminal Background check.

• Attend regular staff meetings

• Maintain confidentiality

Job Skills:

• Ability to provide for the individual needs of clients

• Ability to locate and utilize resources for all individuals

• Basic computer skills

WORK ENVIRONMENT:

The typical work environment is in an office setting; in client’s apartment or common locations including

shelter’s and places not meant for human habitation.
