Position: Case Manager

Department: Housing- UJIMA STARS – FHP-EHI

Reports To: Director of Homeless Youth Services

FLSA Code: Exempt

Hours: 40 hours per week (specific hours are flexible)

GENERAL ACCOUNTABILITY

Provide intensive case management services to individuals and families in the program.

SPECIFIC ACCOUNTABILITY

1. Meet with adult family members monthly, or as needed.

2. Develop and implement a service plan for each individual/family with goals, risk factor and objectives geared toward maximum independence. Ensure that goals incorporate the strengths of the individual/family served. Review the service plan at least quarterly and assess progress toward meeting desired goals. Service plans and case notes should be reviewed and signed off by Program Director.

3. Provide or refer participants to needed services to include: psychiatric evaluation/treatment, mental health services, substance abuse evaluation/treatment, inventory of competencies, skills inventory, employment counseling/placement, medical services, educational/job training resources, vocational training, public legal services and housing resources and referral.

4. Facilitate effective utilization of all community resources to help participants achieve the goals outlined in their service plans.

5. Provide short term crisis intervention to individuals and families.

6. Conduct periodic staffings with other staff members in attendance; to determine service needs and progress toward meeting goals.

7. Complete written documentation on all relevant service issues, service interventions and ensure proper placement in the case file. Program Director reviews and signs off on all said documents. All documents should be completed within 48 hours.

8. Provide follow-up (After-care) services, as needed for up to six months post exit.

9. Actively participate in the agency’s PQI Plan.

10. Maintain a caseload of 15 household at any one time.

11. Work collaboratively with community partners, linking clients and needed services.

12. Assist in collecting and summarizing data for the Quarterly Performance Report.

13. Update job knowledge by participating in educational opportunities, reading professional publications and participating in professional organizations.

14. Document all client interactions in HMIS and E Cornerstone.

15. Work with parents, schools and educational institutions to ensure educational needs of youth heads of households and their children are met.

16. Meet monthly (or as needed) with Rapid Rehousing clients, preferably in client’s home, to access needs and rent portions.

QUALIFICATIONS

Education Experience:

Bachelor’s degree from an accredited university in a human service field.

Training And Skills

One year working with homeless families preferred

Worker Traits:

• Ability to work collaboratively with other staff, service providers and professionals.

• Ability to set limits and intervene appropriately to meet the needs of persons served.

• Ability to utilize trauma informed approach with all clients.

• Conviction about the capacity of people to grow and change.

• Ability to value a nurturing family as the ideal environment for a person.

• Sensitivity to the service population’s cultural and socioeconomic characteristics.

Work Requirements:

• Pass a CANTS (Child Abuse and Neglect Tracking System) check.

• Pass a Criminal Background check.

• Attend regular staff meetings

• Maintain confidentiality

Job Skills:

• Ability to provide for the individual needs of clients

• Ability to locate and utilize resources for all individuals

• Basic computer skills

WORK ENVIRONMENT:

The typical work environment is in an office setting, in client’s apartment or common locations including shelter’s and places not meant for human habitation
